In Bromley we recognise the contributions made by our staff working across health and care services. At the most recent One Bromley Celebration Awards Event, they were recognised for their commitment, resilience, and everything they do on a day-to-day basis for Bromley residents. Winner – Innovative collaboration with Bromley College and young people with special educational needs and disabilities (SEND) to enable them to find job placements and work experience. Plus, the extensive ongoing engagement with families and young people with SEND to inform local services.
Winner – Health clinics run in the Bromley Homeless Shelter over winter providing a range of health care services including health checks, the COVID19 Vaccination and help to register with a GP.
Winner – The national award-winning support package provided to the many care homes in Bromley over the pandemic period and targeted work to help those residents improve their functionality and independence.
Winner – The national award-winning integrated single point of access for discharge, transfer of care bureau and end of life care and home first huddles. These services proactively identify and support Bromley residents being discharged from hospital who need to either continue their recovery in the community or those nearing the end of their life so they can die in their chosen place.
Winner – The integrated post COVID pathway which supports individuals who experience signs and symptoms of COVID longer than 12 weeks. Individuals are assessed and helped to manage symptoms and improve health outcomes.
Winner – The new integrated service for children’s emotional and mental wellbeing, enabling young people to be treated by the right team in the right place.
Winner – The communications campaign to support delivery of the Bromley winter plan, including regular system wide winter pressure information and public leaflets delivered to every household about how to keep well over winter.
Winner – The use of remote technology to deliver a wide range of interactive clinical webinars to staff providing care.
Winner – The extensive Bromley vaccination programme including GP led services, pharmacies, the mass vaccination site, and the wide range of pop-up vaccination clinics set up across the borough to reach different communities.
Winner – The high intensity user service run at the Princess Royal University Hospital which supports people using emergency services on a very regular basis who often don’t need health care, but who have underlying social problems.
