Find out how services in Bromley are working together to deliver more joined up care for residents.

Health and social care services are working together with voluntary services to provide more joined-up, proactive and personalised care for Bromley residents. The name of this integrated care partnership is One Bromley.

One Bromley brings together local NHS health providers, the council, commissioners and voluntary sector to more formally work together to deliver better care for all.

The organisations involved are:

  • NHS South East London Integrated Care Board (Bromley)
  • London Borough of Bromley
  • King’s College Hospital NHS Foundation Trust
  • Bromley Healthcare
  • Bromley GP Alliance
  • Bromley Primary Care Networks
  • Oxleas NHS Foundation Trust
  • St Christopher’s Hospice
  • Bromley Third Sector Enterprise

Working together in a joined-up way means we can coordinate services better. This will help residents to get the help they need when they need it.

We aim to:

  • Improve health and care services which meet local needs.
  • Empower people to take better care of their own health and reduce their dependency on services.
  • Provide support from health and care professionals who act as one team and work for organisations that behave as one system.
  • Improve performance of local services.
  • Reduce duplication. Enable more people to be cared for in the community.

Keep well over winter

Bromley residents are urged to take good care of themselves this winter and seek medical help early when they need it.

To help them, the new One Bromley Keep Well This Winter guide will be delivered to every Bromley home during November and December.   It provides tops tips for staying well and where to go when you need help.  It is available online and in other languages on request.

The guide also signposts residents to where they get more information if they are worried about the rising cost of living, especially heating their homes as the colder months approach.   Advice and information is available from Bromley Well at www.bromleywell.org.uk/our-services/cost-of-living or from their freephone helpline on 0808 278 7898.  Advice and support is also available on the Bromley Council website at www.Bromley.gov.uk/supportingresidents
