In Lewisham and across the country, the NHS is working closely with local councils, voluntary groups, and community organisations to develop and deliver neighbourhood health services, in line with the ambitions set out in the national 10 Year Health Plan.

Neighbourhood working will transform how we support people’s health and wellbeing by bringing more care closer to home and making services easier to access. It will help people live healthier, more independent lives and reduce unnecessary hospital visits by providing joined-up, personalised care.

Neighbourhood working goes beyond treating illness. It reflects the 10 Year Plan’s ambition to shift the NHS from treating illness to proactive prevention and wellbeing support. This means helping people stay well at every stage of life, giving them more control over their care, and addressing the wider social factors that impact health, such as housing, employment, education, and social connections.

Neighbourhood working is not new to Lewisham. We have a strong foundation of integrated local services and partnerships. This next phase builds on that work to deliver the 10 Year Health Plan’s ambitions.

The development of neighbourhood teams in Lewisham has been informed by a range of engagement activity undertaken with partners, local communities and other stakeholders. A group of 16 patients and residents with lived experience of health and care services, were recruited including those with a range of ages, religion, ethnicity, disabilities, and carer responsibilities. The group were invaluable in helping to gather insights and test ideas, shaping and refining our neighbourhood model based on real-life experiences.

Neighbourhood healthcare is not a new idea; we have been working in Lewisham to join up services and better support residents for many years. See two examples below.

Neighbourhood Three Partnership

In 2023, local partners in Lewisham came together with a shared ambition: to strengthen integrated working and improve the health and wellbeing of local communities. Recognising that each neighbourhood is unique, with different people, needs, and challenges, it was agreed to begin by focusing on just one area, learning what works and then sharing that success more widely.

In July 2023 work began on Neighbourhood Three in partnership with Sevenfields Primary Care Network and this continued to evolve over 2024/25.  The partnership includes district nursing teams, adult social care, primary care, public health, and the area Homecare Agency.  Work was also supported by Lewisham Local and the local food network.

Some of the activities already making a difference include: ‘Walk and Talk’ sessions, which are available every Friday and have driven health and wellbeing benefits for attendees including a 67% reduction in GP visits, and well-attended pre-diabetic groups which are helping residents reduce their weight, waist measurements, and achieve consistently lower average blood sugar levels.

There is strong and ongoing commitment from all partners. A dedicated steering group has been established, bringing together a wide range of organisations to collaborate, share ideas, and overcome barriers to progress. This group continues to grow in both size and ambition.

The Waldron Community Hub

In 2024, The Waldron Health and Wellbeing Hub in New Cross underwent a major transformation to create a more welcoming, inclusive space for the community. Working together with partners, the Lewisham Health and Care Partnership created a welcoming space for all, where residents can access local health and care services, find the information and advice they need to support their own health and wellbeing, and connect with their community and friends.

The Waldron is now a thriving neighbourhood community hub which hosts a range of additional services, offering free use of the community space based on local health priorities. The space hosts a wide variety of community groups including: 360 Lifestyle, Bouake Diaspora, Broken Hearted Youth, community kitchen/coffee mornings, Empower Care, Imago Community, Lewisham Council – shared lives and fostering advice, Red Ribbon, Department of Work and Pensions and Citizens Advice Lewisham.

Following early stakeholder engagement work in 2023/4 it became clear that there was an appetite to embed and strengthen integrated working within Lewisham. Recognising that each neighbourhood is different in terms of demography, and each has unique challenges, it was agreed to initially focus on one neighbourhood.  Beginning by identifying the real challenges and adopting an integrated way of working to address these, before taking the learning and scaling this up across the borough.

  • Neighbourhood 2 (covers Blackheath, Ladywell, Lee, Lewisham Central, Rushey Green)
  • Neighbourhood 3 (covers Catford South, Downham, Grove Park, Whitefoot)
  • Neighbourhood 4 (covers Bellingham, Crofton Park, Forest Hill, Perry Vale, Sydenham)

Neighbourhood teams

Developing Neighbourhood Teams is essential to delivering personalised, joined up and localised care across south east London. 

Neighbourhood teams will go beyond multi-disciplinary working by fully integrating staff from health, social care and voluntary services into a single local team, responsible for delivering seamless and coordinated care in a specific neighbourhood. They will work closely together to provide joined-up, personalised support shaped by the unique needs of the communities they serve.

Our aim is to make care more connected, easier to access, and better coordinated so it responds not just to clinical needs, but to the full range of a person’s physical, mental, emotional and social needs.

What the neighbourhood health service will mean for you

  • Joined-up care: different health and care services work together to support all your needs.
  • Easier access: services will be available closer to home, reducing travel and waiting times.
  • Better ongoing support: you see the healthcare professionals from the same team more often, leading to better follow-up care
