Lewisham Health and Care Partnership aims to achieve a sustainable and accessible health and care system, to support people to maintain and improve their physical and mental wellbeing, to live independently and have access to high-quality care, when they need it.

Health and Care Partners across Lewisham have recognised for many years the need for change within our local health and care system. Lewisham’s population is growing, people are living longer – many with a number of long-term health conditions – and the demand for care is increasing, both in numbers and complexity.

Working together, we want to achieve a sustainable and accessible health and care system to better support people to maintain and improve their physical and mental wellbeing, to live independently and to have access to high-quality care when needed.
