Lewisham’s Proactive Ageing Well Service nominated for HSJ Patient Safety Award

Congratulations to Lewisham’s Proactive Ageing Well Service (PAWS), which has been nominated for a HSJ Patient Safety Award in the Improving Care for Older People category.

Created in collaboration with Lewisham and Greenwich NHS Trust, PAWS is an innovative service to support older adults with complex needs. Collaborating with Population Health, PAWS proactively identifies people at high risk of hospital admission, prioritising early intervention through a Comprehensive Geriatric Assessment (CGA). Unlike disease-specific models, PAWS adopts a person-centred approach, involving families or carers where appropriate. The CGA informs personalised care plans, connecting people into appropriate community and secondary care services. Integrated within Adult Community Services, PAWS bridges primary, secondary and community care, improving outcomes for older adults while reducing strain on the healthcare system through its proactive, preventative approach.

Since its launch in November 2024, PAWS has demonstrated measurable improvements in care, integration across services, and overall safety for older adults living with frailty.

The 2025 HSJ Patient Safety Awards will be held on 15 September, where winners will be announced.
