We are delighted to share that Lewisham and Greenwich Trust (LGT)’s Urgent Community Response (UCR) team has been shortlisted in two categories at the HSJ Patient Safety Awards 2025:
This highlights the team’s outstanding work in collaboration with the Lewisham Health and Care Partnership to support people in Lewisham to live healthier, more independent lives.
The Deteriorating Patients and Rapid Response Initiative of the Year Award celebrates teams that have improved processes and outcomes for patients at risk of deterioration. This includes early recognition of conditions such as sepsis, heart failure or kidney injury, alongside clear pathways to get patients the right care quickly.
The Community Care Initiative of the Year Award highlights innovative approaches that improve patient safety closer to home, reduce risks and strengthen care pathways. It recognises projects that ensure people can receive safe effective care in the community, avoiding unnecessary hospital admissions and improving continuity of care
The UCR team provide a vital role in Lewisham, providing urgent care to people at home when their health suddenly deteriorates. By responding quickly in the community, the service helps patients avoid unnecessary hospital admissions, reduces pressure on the emergency services and ensures people get high-quality care closer to their homes.
The HSJ Patient Safety Awards celebrate the best practice in healthcare, judged by a panel of senior leaders and patient safety experts. Winners of the award will be announced later this month.
