Lewisham welcomes first integrated neighbourhood team!
We’re delighted to welcome six new colleagues this week, all of whom are part of our first integrated neighbourhood team (INT). They join two INT Community Link Workers who started on the 18th August. Further colleagues are set to be in post by early November, which will take the team up to sixteen members.
This team will work in partnership with colleagues across the system and will add to the Neighbourhood Coordinators already in place in the borough. Their work will build on existing initiatives including Multi-Disciplinary Meetings (MDM) and the Positive Aging Well Service (PAWS).
Welcoming these colleagues is an important milestone for our neighbourhood journey. Our first integrated neighbourhood team (INT) will support people living with multiple long-term health conditions. They will initially focus on those living with cardiovascular disease, ensuring better continuity of care and delivering more care closer to home.
