North Lewisham PCN wins HSJ Award

Congratulations to the North Lewisham Primary Care Network and Red Ribbon Living Well, for winning the HSJ award in the Primary and Community Care Innovation of the Year Award.

They won for their year-long project to reduce health inequalities in the community, entitled Health Equity Partnership: A symbiotic approach to tackling health inequalities.

The partnership brought NHS teams and local organisations together to make health services easier to access. Here’s what they achieved:

  • Health equity and wellbeing champions – Local champions were trained to spot health needs in communities most at risk.
    Community health hubs
  • Nine hubs were set up as one-stop events – They offered NHS health checks and specialist screenings to 678 people. These hubs were run with voluntary sector partners to bring care closer to home.
  • HIV stigma training – The team co-designed education sessions and animated resources to help reduce HIV-related stigma in primary care.

This award celebrates the power of collaboration to improve health and wellbeing for North Lewisham’s residents.
