One Bromley Hospital at Home team invited to share learning at London summit

Colleagues from the One Bromley Adults’ Hospital at Home service recently presented at the London Region Virtual Ward Summit, hosted by NHS England. The event brought together NHS teams and partners to explore how virtual care can improve outcomes and support more integrated neighbourhood services.

Hospital at Home is a collaborative, integrated model of care that delivers a range of monitoring and interventions in the community that would otherwise be delivered in an acute hospital. It helps to prevent unnecessary admissions and supports earlier, safer discharge. In Bromley, the model supports patients with complex needs, including respiratory conditions, frailty, palliative care, and those requiring intravenous (IV) therapy such as antibiotics, fluids and diuretics. Remote monitoring is also used to provide safe, responsive care in the community.

At the summit, Lorna Redpath, Head of Hospital at Home (Bromley Healthcare), and Ruby Yates, QI and Research Lead (Bromley Healthcare), presented an overview of the Bromley model and shared learning on how the team is delivering hospital-level care at home. Between April 2024 and March 2025, the service saved over 10,980 hospital bed days, while enabling more patients to recover in familiar surroundings with one-to-one clinical support. The team also discussed how remote monitoring is being used to identify deterioration early and support clinical decision-making.

The service has been recognised nationally and internationally for its impact on both patients and the wider system. In March 2025, the team was invited to present at the World Hospital at Home Conference, with contributions from Dr Lynette Linkson, Clinical Director for Hospital at Home (King’s College Hospital NHS Foundation Trust), Dr Sovrila Soobroyen, Lead GP (Bromley GP Alliance), and Fiona Hodson and Dr Joy Ross (St Christopher’s Hospice). In 2024, the model won a LaingBuisson Award for Best in Primary Care and Diagnostics, and Dr Linkson was shortlisted for Clinical Leader of the Year at the HSJ Awards.

The presentation was extremely well received for contributions to regional learning and highlighted how local partnerships are enabling safe, effective care at home.
