• INT structure and membership: Define the types of services which will span neighbourhoods, and in some cases, systems and how these will be implemented (including working across organisational boundaries).
• Common care frameworks for key population segments: Develop a common framework and model of care for three priority cohorts.
• Integrator function: Identify and support integrator arrangements developed at place / neighbourhood to ensure they provide additional support and options for parts of the system facing challenges, including primary care practices.
• Clinical governance: update clinical governance and risk management to ensure good governance of neighbourhood working, enabling effective quality assurance and regulation

• Single PHM approach: Develop a single population health management function for the ICS to ensure INTs are well-equipped to segment their population and use this to inform service delivery and resource allocation.
• Outcomes framework: Define a shared outcomes framework that reflects INT priorities including defining the data sources that will support delivery of the framework.
• Evaluation: Ensures a clear and consistent framework for measuring impact & learning of INTs, enabling continuous improvement.
• IG / Information sharing & digital enablement: Ensure there is an effective integrated data repository and digital tools to inform proactive health planning and support self-empowerment moving beyond purely health to encompass wider determinants of health – addressing barriers to effective information sharing such as inconsistent governance frameworks, data privacy, and technological fragmentation and aligning with segmentation approach.

• Workforce: Enabling the ability for neighbourhoods to have effective clinical governance with a shared workload & integrated workforce planning with a focus on shifting and expanding the workforce “upstream” into proactive care through recruitment, skills development, and making full use of the depth and breadth of professionals and experts -by-experience within local areas.
• Organisational Development: A shared approach to neighbourhood, place and system culture change, learning and quality improvement, strengthening mechanisms for sharing best practice across neighbourhoods and places to support the test and learn approach and ensuring staff on the ground are ready and able to shift ways of working.

Environmental sustainability is a core priority and neighbourhood working plays a vital role in helping to achieve net zero ambitions.

The South East London ICS Green Plan (2025–2028) recognises neighbourhoods as a key delivery mechanism for sustainable, joined-up care. By shifting services closer to home, reducing duplication, and designing out waste, neighbourhood working supports both better health outcomes and environmental stewardship.

Sustainability is now embedded within the Neighbourhood Outcomes Framework, with specific goals and activities linked to environmental impact,  including active travel, community-led initiatives, greener service design, and workforce training in sustainability.

As we continue to develop neighbourhood models of care, we are committed to ensuring that environmental sustainability is not just an add-on, but a fundamental enabler of healthier communities and a healthier planet.
