In Bromley and across the country, the NHS is working closely with local councils, voluntary groups, and community organisations to develop and deliver neighbourhood health services, in line with the ambitions set out in the national 10 Year Health Plan.

Neighbourhood working will transform how we support people’s health and wellbeing by bringing more care closer to home and making services easier to access. It will help people live healthier, more independent lives and reduce unnecessary hospital visits by providing joined-up, personalised care.

Neighbourhood working goes beyond treating illness. It reflects the 10 Year Plan’s ambition to shift the NHS from treating illness to proactive prevention and wellbeing support. This means helping people stay well at every stage of life, giving them more control over their care, and addressing the wider social factors that impact health, such as housing, employment, education, and social connections.

Neighbourhood working is not new to Bromley. We have a strong foundation of integrated local services and partnerships. This next phase builds on that work to deliver the 10 Year Health Plan’s ambitions.

In Bromley we will work cohesively together in our neighbourhoods to deliver a holistic person-centred experience with a focus on prevention and support closer to home.

We will achieve this by:

  • ensuring our knowledge of the whole person, the strength of their social networks, and the support, services and inequalities present in their communities, is at the centre of our proactive planning.
  • our commitment to providing joined up health, social and voluntary sector support that delivers better health and wellbeing for our residents.

Neighbourhood healthcare is not a new idea; we have been working in Bromley to join up services and better support residents for many years. See two examples below.

Bromley Child Health Integrated Partnership (BCHIP)

BCHIP is an award-winning multi-agency approach to implementing integrated children’s triage and clinics in the borough. This model, which rolled out across Bromley in 2024, offers a new way of providing a network of Bromley services, drawing in the many health, care, and support services families and children need, facilitated by a multi-disciplinary team.

It aims to reduce hospital admissions and attendances and empower families to better understand and manage their child’s wellbeing. This means children and families can access the services they need closer to home. Fewer children need to attend hospital outpatient clinics and can be seen much sooner, by the right clinician and service for their needs.

The impact of the service has been significant, with patients receiving services up to 33 weeks faster through BCHIP. Early evaluation indicates that children attending triage have 21% reduced attendances at primary care for the following six months, and those attending a clinic have a reduced attendance by an average of 61%.

Feedback from families has been overwhelmingly positive, highlighting the benefits of the community clinic model including the longer appointment times and the convenience of seeing specialists at their GP surgery.

The One Bromley Wellbeing Hub

The One Bromley Wellbeing Hub which is located in The Glades Shopping Centre was formally launched by the Mayor of Bromley on 9 July 2024. It brings together health, care, and voluntary services to work in a joined-up way for local people and aims to reduce inequalities in the borough by making these service freely available and easily accessible by all.

The Wellbeing Hub hosts several regular services including Vital 5 Checks, Bromley Well and Citizen’s Advice, the Stop Smoking Service, maternity vaccinations and maternity stop smoking support and sickle cell community care.

  • South East (covers Orpington and The Crays)

  • South West (covers Hayes Wick and Five Elms)

Neighbourhood teams

Developing Neighbourhood Teams is essential to delivering personalised, joined up and localised care across south east London. 

Neighbourhood teams will go beyond multi-disciplinary working by fully integrating staff from health, social care and voluntary services into a single local team, responsible for delivering seamless and coordinated care in a specific neighbourhood. They will work closely together to provide joined-up, personalised support shaped by the unique needs of the communities they serve.

Our aim is to make care more connected, easier to access, and better coordinated so it responds not just to clinical needs, but to the full range of a person’s physical, mental, emotional and social needs.

What the neighbourhood health service will mean for you
