Population Health Management – explained

Population health management (PHM) involves using data we hold about our patients and communities and their health requirements to plan how we deliver the best, tailored care for our local population. Its applications range from the personal, to the population level. In order to build momentum around the adoption of PHM in south east London, we have to make sure people understand what it is, and share best practice in how to apply it.
