South east London hub of London Care Record shortlisted for the 2025 HSJ Digital Awards

We are pleased to announce that the South East London hub of the London Care Record has been shortlisted in the Connecting Health and Social Care through Digital category at the 2025 HSJ Digital Awards.

The awards celebrate innovative digital projects that transform care, improve efficiency, and enhance patient outcomes.

South east London’s vision is to provide the most joined-up care possible. Having access to rich, wide-ranging data about the person being treated or cared for is a key priority.

This ambition – developed in partnership with all six south east London local authority adult social care services, alongside acute, mental health, community services, care homes, community pharmacists, GPs, and broader London partners – was to create a unified, secure, and longitudinal view of an individual’s health and care information. This vision was realised through the London Care Record (LCR), designed to seamlessly connect health and social care providers across diverse settings and organisations.

The LCR includes information such as conditions, test results, medicines, allergies, care plans, and hospital discharge summaries.

The LCR helps health and care professionals provide joined-up care, while saving valuable time. In January 2025 alone, the SEL hub of the London Care Record was accessed 690,000 times by SEL clinicians—the second highest usage by any system in England.Across London, the LCR is used around two and a half million times a month by over 100,000 different health and care staff.

This supports safer, more effective, and faster care across the capital and beyond. It saves time for clinicians gathering information and improves the patient experience. Patients no longer need to repeatedly tell their story to different care providers, unnecessary investigations can be avoided, and hospital discharges are smoother, as pharmacists safely prescribe ‘Take Home’ medications using GP-held information.

Michael Bamberger, Performance Operations Manager at the Royal Borough of Greenwich Adult Social Care Services said: “We want to make sure our residents get the best care possible. To do this well, it is important that the professionals helping them can access the right information at the right time and place. The London Care Record is making this possible and we’re pleased to hear that this work is being recognised for the impact that it has had for our staff; it means assessments have instantly available up to date information rather than having to request information from GPs.”

Emily Gibbs, Southwark GP and Primary Care Care Homes Lead said: “The London Care Record shows how we can all benefit from sharing data. The London Care Record views provide effective support for attending outpatient appointments and for actions from hospital care episodes for our practice team. The team in my practice is spending 25% less time on unnecessary investigations and responding to requests for information as it is being shared automatically. Faster information to support clinical decisions and less time spent manually sharing information; what’s not to like?!”

We have ambitious plans to go further and faster with the London Care Record and look forward to extending access to our children’s social care teams and further care homes.”
