The Lewisham Health and Care Partnership team are delighted to announce the launch of Virtual Ward Plus, an innovative prevention service that will help residents to live independently and manage their health with confidence.
The borough team are working in partnership with Doccla to provide the proactive care service for people living with respiratory conditions, such as asthma or COPD. The service will be available to those who have been identified by the NHS clinical team as high risk of complications and may benefit from additional support.
By using simple technology that enables remote monitoring, the service helps the local NHS team spot small changes in health conditions early. This enables support to be provided sooner, helping residents stay well and avoiding unnecessary visits to the hospital.
Eligible residents who join the programme will receive dedicated support from a team of health coaches and clinicians, easy-to-use tools to help keep track of their health, and personalised care helping people for more in control of their conditions.
