Community Specialist Palliative and End of Life Care Services

Specialist community services support people with end of life symptoms that cannot be managed in standard community care. Our teams visit people at home or in outpatient settings to help them live well and focus on what matters most to them.

We also give advice to healthcare professionals on managing symptoms at the end of life.

We do not provide respite or personal care.

Bereavement service

Our adult bereavement service supports anyone whose relative or friend was cared for by St Christopher’s before they died. We also support any grieving adult who lives in Bromley or Lewisham, or is registered with a GP in either borough.

Palliative Care

Palliative care can help you, and those close to you, cope with the consequences of life-limiting illness.

We are able to see people living in Lambeth and Southwark who are registered with a Lambeth or Southwark GP.

We are also able to see people in Guy’s and St Thomas’ hospitals and in our outpatient clinics at Guy’s Hospital.

King’s College Hospital

Specialist inpatient palliative care service

The palliative care team at King’s is based in hospital and takes referrals from all wards at the Denmark Hill site.

They support people with serious illness by helping with physical symptoms, emotional wellbeing and practical needs. The team works to improve quality of life for anyone facing a life-threatening condition.

Support is available whether someone is expected to get better or is nearing the end of life. Care includes physical, emotional and spiritual support. The service is open to anyone who needs it.

operating hours: working hours, 9am-5pm 7 days a week (reduced services at weekends / Bank holidays). An on-call service operates outside these times.

Marie Curie

Planned Overnight Service

Planned overnight care provides a healthcare assistant to support people at home. The assistant stays with the person all night and gives the care they need, including any prescribed oral medicines.

You can ask your district nurse, GP or palliative care team to make a referral.
