Care homes in south east London (SEL) provide a home to residents who require support with a variety of needs.
Care homes offer a different environment to hospitals and work in different ways. It is important that professionals in hospitals and care homes work together to ensure people receive coordinated and safe care.
Through engagement with care home staff across SEL, we know that what matters most is being informed, prepared and treated as equal partners. Clear, timely communication makes a real difference to outcomes.
As part of a campaign to improve staff experience and patient outcomes when they are discharged from hospital to care homes from elderly care wards, we have produced some posters which you can download below and share with colleagues.
