A new way of working in neighbourhoods and caring for people with multiple health conditions
On Friday 3rd May, Dr Krishna Subbarayan (Long term Conditions Clinical Lead, NHS South East London ICB) and Rob McCarthy (Associate Director Long term Conditions, NHS South East London ICB) led the webinar “Multi-Morbidity Model of Care: An Innovative approach to Integrated Neighbourhood working”.
The webinar shared innovative practices and presented new approaches to tackle complex patient needs through collaboration across different parts of the healthcare system in south east London.
