This page is aimed at hospital staff, care home staff and other professionals supporting discharge into care homes.

Care homes in south east London (SEL) provide a home to residents who require support with a variety of needs.

Care homes offer a different environment to hospitals and work in different ways. It is important that professionals in hospitals and care homes work together to ensure people receive coordinated and safe care.

Through engagement with care home staff across SEL, we know that what matters most is being informed, prepared and treated as equal partners. Clear, timely communication makes a real difference to outcomes.

The Working Together for Safer Discharge to Care Homes campaign was designed to help hospital and care home staff work better together across South East London. The campaign brought colleagues together to share experiences, discuss challenges and find ways to support safer discharge for residents. Campaign posters, which contain key messages for hospital staff, can be downloaded below.

The campaign highlighted that:

  • Clear communication supports safer discharge.
  • Strong relationships help improve outcomes.
  • Consistent processes can reduce delays and frustration.
  • Sharing learning between organisations helps improve care for residents.
