Community Hospice

Community Specialist Palliative and End of Life Care Services

The community palliative care team supports people with life-limiting illnesses, including frailty. They give advice on managing symptoms, provide end of life care and help people plan ahead.

Specialist nurses work closely with GPs to give this care at home or in outpatient clinics.

The team offers short-term care. They understand that early palliative care can improve both quality and length of life.

They run a seven-day service from 9am to 5pm. You can get advice 24 hours a day.

Marie Curie

Planned Overnight Service

Planned overnight care provides a healthcare assistant to support people at home. The assistant stays with the person all night and gives the care they need, including any prescribed oral medicines.

You can ask your district nurse, GP or palliative care team to make a referral.

Overnight Unplanned Service

Unplanned overnight care is available in Bromley, Lewisham and Greenwich. A registered nurse and a healthcare assistant visit people during the night to give support, advice and symptom relief. They can give prescribed ‘as needed’ injections and manage continuous subcutaneous infusions.

District Nursing End of Life Care - Day service

The Bexley and Greenwich district nursing team supports people and their families at the end of life. They help people live as well as they can and die with dignity.

District nurses give pain relief and manage symptoms for people with palliative conditions. They also talk about future care choices, known as advance care planning. This helps make sure each person’s wishes are followed if they cannot make decisions later on.

The team works closely with partners to meet people’s physical, emotional, social and spiritual needs.

Queen Elizabeth Hospital

Hospital palliative care team (provided by Community Hospice)

We assess people holistically to support palliative and end of life care, with a focus on managing difficult symptoms. We help arrange discharge to the person’s preferred place of care or death, including hospice referrals and Fast Track applications. A hospice social worker also works with the hospital palliative care team twice a week to support patients and families. Our team includes palliative care nurse specialists, a discharge facilitator, a palliative care doctor and a palliative care social worker.
